NEW OFFICE PATIENT
Welcome to The Qi Spot and we are grateful that you have chosen to entrust us with your care. We will endeavor to provide you with the best care possible. We would like you to be prepared and know what to expect for your appointments.
As a new patient, before your appointment, you MUST do the following
1. Make your Appointment at our Patient Portal ONLINE (if you already haven't)
2. Fill out the Qi Spot Intake Form, the Health History Form and the Credit Card Authorization Form ONLINE at our Patient Portal (includes contact information, health history, insurance information, consent to treatment that need to be signed before the appointment). Please fill out the forms to the best of your knowledge. It is crucial for a correct diagnosis at your treatment.
3. It is your responsibility to know your insurance benefits. We can bill insurance for your visits. You are responsible for unpaid visits or balances by your insurance company.
4. During COVID-19 period, we will be taking credit card information online or you can call ahead with your credit card number. 503-919-9091.
We accept same day appointments if there is availability. Please email or call to see if we have an opening.
Appointment Lengths
First Acupuncture visit - 60 minutes long
Follow up Acupuncture appointments - 30 to 60 minutes long
Patients with Medical Insurance
Most insurance companies in the US, especially Oregon, Washington and California cover acupuncture, but health plans vary. We can also check your insurance coverage if you are not able to or not sure.
Currently, our Acupuncturist, Wade, is an In-Network provider for many insurance companies including Regence, BlueCross BlueSheild, Kaiser, CHP, MODA Health, United Healthcare, Cigna, Cigna Greatwest Pacific Source, Lifewise, Aetna, HealthNet, Providence Health Plan and more and all auto accident insurance from patients with Auto injuries. Even though out-of-network, he also takes UMR for Legacy Employees.
Please Note: We do not take Regence insurance that requires pre-authorization unless you get approval from our office. Please check to see if you require pre-authorization before you come in for an appointment. If you plan on using your Regence insurance, you need to wait to receive pre-authorization before you come in for a visit, otherwise you will be charged for the full visit at the time of service.
Patients with Auto Accident Insurance
If you were in an Auto accident within the last year (Oregon Residents) or 2 years (Washington State Residents) and have filed a claim for your accident, and you are experiencing any sort of pain, your Acupuncture treatments are covered 100% as long as your benefits are not maxed out, or you have been authorized by your lawyer to continue treatment.
You do not need a referral from your doctor and you do not need to pay anything out-of-pocket. Just fill in the Auto Accident insurance information with the claim number in your Health History Form and we will verify everything for you.
Self Paying Patients
If you do not have medical insurance that may cover your acupuncture treatments, we will be able to help you through our discounted rates for patients who prepay or pay on the day of service.

Appointment Cancellation and NO SHOW Policy
We recognize that emergencies do occur and people have to reschedule or cancel their appointments (which you can do online or by emailing us) for whatever reason. Most situations can be anticipated and therefore it is courteous to let us know in advance.
Please recognize that scheduling an appointment involves the reservation of time specifically for you, and that consequently, a minimum of 24 hours notice is required to reschedule or cancel an appointment. Unless otherwise agreed to in advance, $30 will be charged for sessions missed without such advance notification or an automatic redemption of a Coupon.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.
Under the law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.
You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
If you schedule a health care item or service at least 3 business days in advance, make sure your healthcare provider or facility gives you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, make sure your healthcare provider or facility gives you a Good Faith Estimate in writing within 3 business days after scheduling. You can also ask any health care provider or facility for a Good Faith Estimate before you schedule an item or service. If you do, make sure the health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after you ask.
If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate and the bill.
